DELETED : DocumentWindow class has been removed.
NEW : File extension association now uses Qt (with QSettings).
NEW : ExtensionManager has a new static method : registerFileExtension(QString).

FIXED : Rename Action::ApplyStatus::ERROR to FAILED. On windows platform with MSVC, a macro ERROR already exists and is conflicting with that one.

      Each launch of an instance of IMP register it for file opening.
      Standard volume image and mesh files type (in the share/testdata directory) are handled.
      See the ImpMainWindow constructor for registered file type.
      Note : file association only works on Windows.

FIXED tentative fix for error http://srv-eccami.ujf-grenoble.fr/viewBuildError.php?buildid=6188 (the word Q_OBJECT appeared in an old comment => camitk_extension macro was triggered unusefully the moc/ui compilation tool chains for nothing as this class is not a Q_OBJECT!). A good practice is to remove old comments from code before commiting; you can always access to previous version via svn and a good commit message...)
